Nomination Instructions

Nominations can be submitted online, by mail or in person.

Online: Nominate online using the Seniors and Elder Service Award Nomination Form below.
By mail: Download/Print a nomination form
and mail it to:

Nomination Criteria

What criteria does the nominee fulfill: 

Nominee is a St. Albert senior/elder (55+) resident who has provided extraordinary contributions or service to the community of St. Albert within the last 5 years. 
Nominee is a person (under 55) who has provided extraordinary service or has given their time and talents to the benefit of a senior or seniors in St. Albert within the last 5 years.

Nominations should include as much detail and information as possible to support the application. Please answer the following questions with this in mind.
